A 5-year review of surgical outcomes for deceased-donor liver transplantation in British Columbia

چکیده
Background: Surgical complications after liver transplantation are known to contribute to morbidity and mortality. Evaluation of complication rates is a crucial part of assessing transplant program performance and identifying areas for improvement. A study was proposed to determine complication rates and outcomes using BC Transplant data. Methods: Patients undergoing deceased-donor liver transplantation from 2007 to 2011 were identified via the prospectively and independently managed BC Transplant database. Both paper and electronic records were reviewed for 191 patients who underwent 201 individual transplant procedures. The records included operative reports, radiology studies, outpatient clinic notes, and all available follow-up information obtained at Vancouver General Hospital until 31 August 2014. Rates of surgical complications and mortality were calculated and compared with published rates from other centres. Results: The retrospective review of records revealed an in-hospital mortality rate of 5.2% and a 1-year mortality rate of 8.4%. Complications occurred in 107 of 201 transplants. Half of all patients (53.2%) experienced a reoperative, biliary, or vascular complication. The most common complications were a planned or unplanned reoperation (32.3%), a bile leak (9.0%) or anastamotic stricture (16.9%), and hepatic artery thrombosis (7.5%). Conclusions: Rates for 1-year inhospital mortality, biliary complications, and hepatic and portal vein thrombosis in deceased-donor liver transplantation in BC were similar to rates found in the literature. Rates for hepatic artery thrombosis appeared higher than those reported in the literature and further investigation is needed to identify factors that may be contributing to this complication in transplant recipients. Review of Natural History, Benefits and Risk Factors Pediatric Liver Transplantation
Liver or hepatic transplantation (LT) is the replacement of a diseased liver with part or whole healthy liver from another person (allograft). Human liver transplants were first performed by Thomas Starzl in the United States and Roy Calne in Cambridge, England in 1963 and 1967, respectively. Liver transplantation is a viable treatment option for end-stage liver disease and acute liver failure....

BACKGROUND The purpose of this study was to summarize the outcomes we achieved using deceased-donor liver transplantation (DDLT) in the past 10 years at Chang Gung Memorial Hospital-Kaohsiung Medical Center (CGMH-KMC). METHODS Between March 1993 and March 2003, 53 DDLTs were performed at CGMH-KMC. Patients were divided into 2 stages: stage 1 (n = 22) from March 1993 to February 1998, and stag...
متن کاملMucocele mimicking a gallbladder in a transplanted liver: A case report and review of the literature
Biliary mucoceles after deceased donor liver transplantation are a rarity, and mucoceles mimicking a gallbladder from the recipient remnant cystic duct have not been described until this case. We describe a 48-year-old male who presented with right upper quadrant pain and was found to have a recipient cystic duct mucocele 3 mo after receiving a deceased donor liver transplant. We describe the c...

The use of extended criteria donors is an effective way of reducing the shortage of deceased donor organs. Evidence of significant liver trauma in a deceased donor usually rules out transplantation of the liver. The aim of this study was to evaluate the use and outcomes of donor livers with preprocurement trauma. Records of all 312 deceased donors with a history of trauma between January 1986 a...
متن کاملShort- and long-term outcomes with the use of kidneys and livers donated after cardiac death.
The shortage of deceased donor kidneys and livers for transplantation has prompted the use of organs from donors deceased after cardiac death (DCD). We used the UNOS database to examine patient and graft survival following transplantation of DCD organs compared to those following grafts from donors deceased after brain death (DBD; for livers, grafts from donors < 60 years old were labeled '< 60...
